A. Arrange urgent hospital transfer for IV antibiotics, CT KUB, and likely ureteric intervention

B. Administer diclofenac 75mg IM, prescribe tamsulosin 0.4mg daily, and arrange outpatient review in 48 hours

C. Prescribe trimethoprim 300mg nocte for 7 days and arrange outpatient CT KUB within 48 hours

D. Commence tamsulosin 0.4mg daily for medical expulsive therapy and arrange renal ultrasound next week

E. Administer paracetamol 1g IV, withhold irbesartan, and encourage oral fluids with GP review tomorrow
